Antioxidant Effects Of Tiva And Isofluran Anesthesia During Breast Cancer And Benign Thyroid Surgery

Objective: The aim of this study was to evaluate the antioxidant properties of propofol comparing with isoflurane which didn’t have antioxidant effects, in non-abdominal surgery and oncologic surgery where an oxidative stres is present.
Material and Methods: 54 ASA I-II patients enrolled the study. The study groups were; group I (breast Ca-propofol, n= 13); group II (breast Ca-isoflurane, n= 16); group III (nodular goitre-propofol, n= 13); group IV (nodular goitre-isoflurane, n= 12). Blood samples were taken preoperative, peroperative 1th hour and postoperative 6th hour for evaluating plasma and erythrocyte malonedialdehyte (MDA) levels and erythrocyte catalase, superoxide dismutase (SOD) and gluathion peroxidase (GSH-Px) levels and uric acid, total protein, albumin, trigliseride, cholesterol, VLDL, LDL, HDL plasma levels which thought to be natural antioxidants. Varyant analysis and Chi-Square test were used for statistical analysis.
Results: Demographic data was similar in all groups. There was an decrease in plasma levels of MDA in the propofol groups and it was clear in breast Ca group when compared with the isoflurane groups at the peroperative first hour (p< 0.05). In the same interval the MDA levels of the erythrocytes were increased in isoflurane groups and decreased in propofol groups (p< 0.05). There were no differences in erythrocyte SOD levels among four groups while the catalase and GSH-Px levels were decreased in propofol groups at the first hour of the operation (p< 0.05).
Conclusion: Our results suggested that propofol may have antioxidant properties when compared with isoflurane in patients undergoing surgery by preventing lipid peroxidation and supporting the hypothesis of enzyme mediated antioxidant defence mechanisms. Propofol decreased plasma MDA levels and this effect was clear in breast cancer group. Therefore, propofol may be a preferable agent especially for cancer surgery in the future.
